UMW Cross Country and Track & Field Camp

A full-time certified athletic trainer will be on call. All athletes must provide proof of medical-accident insurance.

I authorize the camp staff to instruct and supervise my child’s participation in the camp activities. I understand that there is a risk of accident or injury during these activities, so I therefore grant permission to the camp staff to attend to any minor injuries or contact the appropriate medical professionals if needed. I also understand that any possible medical costs associated with such injury are my personal responsibility. Also, by checking the box below, I agree to fully absolve The University of Montana Western and The Montana University System, all of their employees or volunteers, from all liability for any damages, injuries, or losses which might be sustained while participating in the program.
